The entries are made in rough chronological order determined by the date of gazette.

The register was created by the following agencies:
- City Engineer and City Surveyor's Department II (AG-0088) in the period 17/03/1925 to 29/08/1928;
- City Engineering and Building Surveyors Department (AG-0061) in the period 31/08/1928 to 31/05/1936 and
- City Engineers Department I (AG-0005) in the period 01/06/1936 to 26/09/1986.

The recording of alignments initially constituted a section of the Register of Acquisitions (AS-0114). This section was terminated and a new register created to deal with expanding acquisitions for this purpose. Information recorded in the Register of Acquisitions was transferred to the Realignments Register, with entries backdated to 17 March 1925.

This register is divided into various sections. The first section is on pages 1 to 12, dates from 17 March 1925 to 26 September 1986, and covers realignments 1 to 148. The second section is on pages 50 to 84, dates from 19 June 1928 to 26 May 1931 and covers realignments 1a to 197b. This whole section is marked as 'rescinded vide various resolutions of Council - cases to be reconsidered as they become necessary'.

The third section is on pages 100 to 158 and it contains coloured plans of subdivisions and realignments for the following: Rose Hall subdivision; realignments in Roslyn St, Cleveland St, Pelican St, Hunt St, Purkis St, Pelican Lane, Oswald Lane, St Neot Avenue, Springfield Ave, Salisbury Rd, Judge St, Ultimo Rd, Royston St, Fitzroy and Riley Streets, Onslow Ave, Paints Lane, Moorgate Lane, Mackey St, Lambert St, Layton St, Kellett St, Kirketon Lane (now Farrell Ave), Kings Cross Rd, Ithaca Rd, Harbour St, factory St, Hands Lane, Goodchap St, George St (now Broadway), Goulburn St, Elizabeth Bay Rd at Ercildoune, Elizabeth Bay Rd, Darlinghurst Rd, Craigend St, Balfour st, Billyard Ave and Ithaca Rd, Bayswater Rd and Barcom Ave, Rosebank Ave and Premier Lane, Elizabeth St, York Lane, Little Riley Street, Goulburn St, Dixon St, and Devonshire St. This section has contents dating from 20 January 1919 to 10 December 1951.
